New Hire Enrollment: You have 30 days from your hire date to elect your Lubrizol benefits. The benefits you elect are effective from your hire date through the end of the plan year unless you have a qualified family status change or other qualifying event. If you do not enroll for benefits within 30 days of your hire date, you will not have medical/Rx, dental or vision coverage during the current plan year; you will be eligible to receive Long Term Disability coverage equal to 60% of eligible base pay; and basic life insurance coverage of 2x your eligible pay.
Follow these steps to enroll in coverage:
- Get familiar with Lubrizol's benefits by exploring our Important Enrollment Resources (below).
- Attend “New Hire Benefit Orientation,” that is assigned through Career Essentials Learning.

- Your social security number and the social security numbers of any family members you wish to add.
- Your date of birth and the date of birth for any family members you wish to add.
- If you are adding a domestic partner to coverage, you will need the required supporting documentation.
- Visitar Go.Lubrizol.com/BenefitsEnrollment to enroll and add your eligible family members to coverage.
Supporting documentation may be requested by Alight our dependent verification services provider. Examples of supporting documentation may include marriage certificate, domestic partner affidavit, child(ren)'s birth certificates, etc. Information about the dependent verification process, including a unique participant ID number to include on all correspondence will be mailed to your home via USPS after your elections have been made.
Obtaining ID Cards:
- Typically, ID cards are mailed by UMR, Caremark and EyeMed when new benefit elections are made or if a dependent is added to existing coverage as the result of a qualified life event. (marriage, birth etc.) Cards are usually mailed within two weeks of electing or changing coverage. While physical ID cards are being mailed, participants can login to the vendor site to print temporary ID cards.

- Caremark: Log into Caremark here http://caremark.com/ to either register for an account or sign into your existing account to order an ID card or print a temporary ID card.
- EyeMed: Log into EyeMed here http://eyemed.com/ to either register for an account or sign into your existing account to order an ID card or print a temporary ID card.
- MetLife does not automatically issue ID cards. Participants can order them by logging into MetLife here http://metlife.com/dental to either register for an account or sign into your existing account to order an ID card or print a temporary ID card.

All regular, full-time U.S. salaried and non-union hourly employees are eligible to enroll in benefits offered through Benefit Essentials. Regular part-time employees working at least 20 hours per week and those employees participating in a phased-in retirement program are also eligible.
U.S. wage employees whose collective bargaining agreement establishes eligibility for this program may participate.
Eligible Family Members
You may enroll your eligible family members including:
- Your spouse
- Your domestic partner*
- Your domestic partner’s children to age 26
- Your children to age 26
- Dependent children over the age of 26 who are incapable of earning a living because of a disability that was in existence at the time they would have no longer been eligible for coverage under the plan
Eligible children include:
- natural children
- foster children
- adopted children
- step children
- court-ordered children
- children for which you are legal guardian
- domestic partner’s children

*Domestic partner eligibility is subject to certification and must be completed within 30 days to receive coverage for your eligible domestic partner and their eligible children.
